    Hi There!  I am in the process of collecting as many forms as I can from both jails and prisons from as many different states as possible.  Our goal?  To Reduce Paperwork and Improve Correctional Health Care.  Are you tired of people prattling on endlessly saying nothing. or worse, actually writing nothing legible at all?  Let's all communicate more efficiently.  Actually, the goal of the survey is to help standardize clinical forms in a safe, medical-legal manner that protects the clinician, the institution, and the client.  

    So, if you are a psychiatrist, psychologist, social worker, therapist, OT, PT, nurse, pharmacist or know of any out there, PLEASE< PLEASE< PLEASE, send us your forms.  Over the next 6 months, my colleagues and I will be compiling them, positing them on a blog, getting your feedback on the forms, and publishing the fillable master-forms in a book by APPI press. The reviewers will include nationally published authors and prison experts who are frequently appointed by the federal government as monitors.

     My favorite part is that the forms will also include correctional-officer input forms, dedicated to what we know YOU observe.
