    Our FTO's currently complete a Daily Observation Report on their assigned trainees. I am looking for a similar report/evaluation that a supervisor would complete on a weekly basis as a followup to the daily reports. If anyone has anything of this sort and is willing to share, it would be greatly appreciated.
